We are probably about a month away from sending our application in for my US citizen husband.

I have a question - do I *HAVE* to send the medical with the application? Or could we send it at a later date, do they request it?

You're supposed to complete the medical before you submit the application and include proof that the medical has been complete along with the application.

If you don't do the medical in advance as instructed, this will likely negatively impact the processing time of your application.
 
I kind of figured that was the case as it's all I've been reading here, but this quote on CIC's website confused me:

"Instructions on how to take the medical examination will normally be sent to you after you submit your application to the visa office. "

That for non-spousal family applications (parents / grandparents). For spousal, the medical is supposed to be done in advance. (I know the info on the CIC site can be confusing.)

e. The DMP will give you (original) copy 2 of the medical report that you need to include with your application. copy 1 will be sent by the DMP to the appropriate medical office for processing.
