Hi

I lost my Upfront Medical report given by Doctor when i had my medical exam. Now, I called them to give me another one. They said, they can't send it again because they already submitted it with CIC.

I have not submitted my documents yet. I can't miss medical report field because it's in my checklist. I don't want go for another Medical Exam now.

What should i do? Any suggestions?

they should provide you for extra bucks... Just ask them to reprint information sheet or IMM1017(forgot exact form no) for you.

ahh..terrible buddy. Visit the doctor, explain your situation and ask them for e-Medical information sheet. They should be able to reprint it
for you. All the best !!
