The receipt for medicals (which is completed by the physician) is supposed to be enclosed in the application package. So I think the correct answer is yes, at least it was for our outland application.
 
It depends on if you are applying in family class or as skilled worker.

family class should be done before applying and skilled work is done after applying when the IO requests it.
 
Please am confuse . what about for express entry, i guess if you do when submitting and pass ,you dont have to do it again .is it?

Medicals are valid for a year. Sometimes they waive the requirement for a 2nd medical if it is slightly over a year.
 
It says right in the package (For outland Family sponsorship, at least for me - US to Canada) that if you have NOT had a medical done to wait until they request one. If you DID have one done, then follow the instructions on the package about sending it in. They're valid for one year.

But if you have NOT had it done, then wait till the CIC requests the medical.
