G good boy Hero Member Feb 17, 2017 483 67 Jul 5, 2017 #4 Am pretty sure you won't be asked for it. If cic needs it, they can always find it by themselves. Just to be on the safer side, contact your medical test center and tell them your situation. They are normally very cooperative.
