Can Someone help me with the below scenario

Received ITA and all the documents are done and have no clue of how to deal with the medicals?
Should I directly take an appointment for the medicals? I would be accompanied by my wife and my son.

In the mandatory documents for Medicals it was mentioned as the Medical Report form (IMM 1017E), if you do not get an upfront medical exam. We will send you this form

I mailed the CIC for this Medical Form and automated response was not satisfactory. Can I directly take an appointment or suggest me a right contact to get that medical report form?

Go make an appointment. Once your medical is done, doctor will give you an information sheet which you then upload.
Medicals are done upfront for EE. No need to contact CIC for anything.
