Hi everyone,
My spouse got her medicals done in July 2014. Our file was transferred to NDVO in January 2015. Our ECAS status never showed any information about her medicals. It only showed file received on Oct 22nd 2014. My wife contacted NDVO regarding this but did not get a response. I ordered the GCMS notes and this is what it said:

Medical report provided, although unable to associate with file due to name match glitch. For visa office to review.

Then today my wife got an email from NDVO asking her to do her medicals from a list of panel physicians and send within 30 days. They have attached the medical request form IMM1017E
to the email too.

My concern is the UCI and Application# on this email is different from what we have been getting so far. Even the IMM1017E has the wrong UCI.
Has anyone seen this happen before? If yes, what should be our next steps?

Reply to the visa office with details of the medical exam she did (name of Panel Physician, date, scanned copy of any documents you have). They should use that to look up the medical exam results and link everything up.

Thanks for the reply. My wife already did send them an email with all the details you mentioned above. She also sent the document she received from the physician. This was about a month ago and they never got back to her. Also redoing the medical won't be a hassle, just that why does this document have a different UCI and file number.

I don't have enough info to anser.

The letter is different from which prior UCI? That from the AOR, a prior visa...? Each person usually only has one UCI so it may indicate an issue you do want to uncover to make sure that they have her linked up properly and the request really is for her.

If you reply directly to the email without changing subject, etc. in most VOs the e-mail gets routed directly back to the person reviewing your file so if you send them another e-mail asking about the different UCI and file number and about whether the request to redo the medical is for her or just a matter of not having the results having been linked up you should be able to get an answer from someone familiar with her file.

Since you mentioned a name glitch it might be that there is another applicant with the same name. That would explain the different UCI number, since it could be the number of the other person with same name. I suggest contacting CIC about this and explaining that the received document probably has the UCI of someone else with the same name. Then provide them with the correct UCI that you have on all your previous correspondence/documents from the CIC and ask how to proceed from here on.
