Correct, as per my understanding you take the form to your doctor and he/she will do the rest.

Correct, as per my understanding you take the form to your doctor and he/she will do the rest.

Mine didn't even need the form, apparently all they need is your UCI.

I would obviously still take it, just in case they do require it.
