Hi everyone, I have already left Canada a week ago, but today I got my visitor record approval with the medical request. Should I still do the medical exam?

Did you stay past the end of your original stay? If so yes you took advantage of extra time in Canada so you need to do the medical. You should had advised IRCC that you had already left Canada. You may have saved yourself some money.
