My wife just had her medical and got the receipt to send in with the application. I just noticed pdf 98kb which is a form for the doctor to fill out. She did not take that with her. Is this new ? I never noticed it before. No one ever told me about this form. Does she need it even though the Dr gave her the receipt for application.
