Medical.. form?

I keep reading that at the medical you receive a form. We did upfront medical, havent sent the app in yet. So my husband went for the physical, then we went and got the bloodwork and xray done. But like.. we didn't receive any form. So I'm confused.

We were given print outs at the doctors. It is to be included with your application. The doctor said that they use it to open the doctors e-medical online. It should have a photo of you on it also.

We didn't get anything like that? We got a receipt, but nothing like that.

He did say he was going to mail us something after the blood work/x-rays came back? Think that's the form?

Yes. Some doctors handle it that way - mine did that as well.

Apparently we did get that form, my husband didn't tell me he'd out his stuff in my immigration folder, sigh! Thanks everyone Now we're just waiting on the FBI bit. It's been weeks.. Everything else is ready to go. Arggg
