Medical form for dependant

I have sponsored my father (principal applicant) and my mother who live in India (New Delhi). Last week they received notification to go for medical exam, however, the email had only one IMM1017E for my father with all the details filled in. I am confused for my mother....I know she should also take the medical exam but since there is no form for her how would she do it?
