hi,
If I do the medical test what do I need to get from the doctor? and what do I need to say if its for Common Law Sponsorship!

and do need to get this from CIC if HOW???

When you go to your appointment, you must bring:

-proper identification (you will need at least one document with your photograph and signature, such as a passport, driver’s licence or national identity card)
-eye glasses or contact lenses, if you wear them
- any medical reports or test results that you have regarding previous or existing medical conditions, and
the Medical Report form (IMM 1017E), if you do not undergo an up-front medical exam. The visa office will send you this form.

and it is done by the doctor in a few hours or do I need to go somewhere else too?


and it es valid for 12 month right?
also if CIC needs longer for proofing the application or do I need to make it again???

You can find a physician here: http://www.cic.gc.ca/pp-md/pp-list.aspx
Just book your appointment and bring your ID, no need for anything else. You will need to check with the specific doctor office if they have blood lab/xray done in his office or will refer you to some other place. Once completed, doctor will give you a form with barcode which you will submit along with your application.
And yes, it is valid for 12 months and it is not uncommon that it will need to be redone if CIC doesn't complete your file in time.

1. you can find panel physician on CIC (in Canada or your country)

2. I did not bring any form except my passport and tell them it is for spousal sponsorship .
I did not receive any thing from dr. office (except the receipt )
so you might not receive any forms, the result would electrically send to CIC.

if you would receive a form , then you can attach with your application.

3. general check up by dr. then you would have blood test and x-ray.
it took me almost 2-3 hr including waiting.

4. it is valid for 12 month.
so you could do medical before you send off app.
VO would request re-medical when they process your file (your medical is expired )
