BOX 7500 SYDNEY, NS B1P 0A9 Dear Name MEDICAL EXAMINATION INSTRUCTIONS REQUIREMENTS Immigration Canada requires you to have a medical examination. To do this, you need to arrange for an appointment with a designated medical practitioner (DMS) on 9th, 10th, or 11th of April 2018. Note: You must have your medical examination performed by one of the doctors. If you do not follow the instructions, you may have to take another medical examination and pay the fees again. The doctor will require all applicants who are over 10 years old to have a chest X-ray. An x-ray is required by children under 10 if there is a history or clinical examination which indicates there is need or if there is evidence of tuberculosis in the family. You will receive instructions from the doctor. You must take with you to the doctor's office: your passport, your client ID# if kn own, your eye glasses, any relevant medical report(s), and four recent passport size photographs showing head and shoulder for each member of the family required to undergo a medical examination. All costs related to the Medical Examination are your responsibility It is possible that you may be asked to take further tests before a final medical decision is made. The costs of these further tests are your responsibility and over and above the cost of the medical examination. MPORTANT: check here if you are being sponsored by a spouse or partner --- Eyeglasses or contact lenses if you use them --- List of medications you are taking Fraud prevention The integrity of the immigration medical examination process is based on verifying the identity of applicants presenting themselves for an examination and ensuring that all official records correspond to the correct individual. DMPs can help to prevent fraud by following procedures when filling out forms, educating their staff about these procedures and incorporating quality assurance processes into their practice wherever possible. A stamp with a unique number is issued to DMPs. This stamp must be used to authenticate each Medical Report completed by the DMP and is also used to indicate that the DMP has verified the identity of applicants. The stamp should not be shared with other practitioners and is to be used only by the DMP to whom it has been issued unless authorized by Citizenship and Immigration Canada. After reviewing your application an immigration officer will make a decision if you should attend an interview. If an interview is required, you will be informed about the date and place. All family members over 18 years of age must attend the interview. An immigration visa is valid for 12 months from the date of the medical examination. Also its validity depends on the expiry date of passports. An immigration visa cannot be extended. If an applicant has not used it during the period of its validity, he/she must apply for permanent residence in Canada once again.
