medical exam is expired and process almost done

I applied for my wife as family class sponsorship March07,2012 and I got the confirmation letter from CIC on May 31,2012 that states they forwarded my file to Ankara embassy. at that time regular waiting time was 5 months. Later they changed to 16 months! and this applies for files since summer 2011!. After Nov 2012 I followed and got news that the process of my wife file is done (by a trustble friend) and they forwarded to the issuing section. But since that I've never heard anything and her medical is expired end of Jan 2013. I just want to know usually how long it takes a full processed file and ready for visa pass the visa issuing step and does she need to redo the medical exam or not?

how long it takes - nobody can tell you.
For the medicals : if it's the only thing preventing CIC to issue the PR, then they might extend the validity, provided that you have done the medicals in the last 15 months. So - don't do anything. If they ask for a medical, you might have to do only one part of it.
If they are ready to issue the PR, they might have asked to extend the medicals already, and you would not know about it... so no point in doing them before they ask.
