Medical exam - Form IMM 1017

As per the checklist in the CIC website, doctors are supposed to provide a summary report form ( IMM 1017). but i didnt get anything expect just a two documents saying medical exam completed form the corresponding clinic. and it says to include it with your application. Please advise.

Do i need that form?

No, you don't need IMM1017; it is no longer issued.

you should have received the "emedical" receipt. this shows CIC you have completed the exam and will provide information about the electronic file the doctor submitted. CIC doesn't use the old medical forms, everything is electronic. make sure your receipt is in your package, and keep a copy for yourself just in case. CIC will be able to find your results through that receipt and connect your results to your file electronically.
