Medical exam for dependent children staying abroad (19y,20y)

we are living in Canada (spouse and 13y daughter) and applied for the pr. We have 2 boys in Germany (19y, 20y). Do we need an medical exam for them even if there are 19 and 20 years and not planning to move to CA at the moment?

They are classified as dependents as long as they aren't married or in common law relationships. This means they must take and pass the medical in order for your application to be approved. You can only exclude them if they are either married or common law (i.e. have lived with their partner for a minimum of one year continuously).

Both boys want to stay in Germany for the next years. One studies and the other one works, so the medical exam doesn't make sense.
Would it be ok when the boys write a letter with refusing doing a medical exam?

No - they need to complete and pass the medicals. This is required in order for your application to be approved.

IRCC may ask for evidence and you should be prepared to provide it. You certainly don't want to say they are common law if it's not true. Then you'll be risking a misrepresentation finding and a 5 year ban from Canada.
