Urine test, blood test, chest x-ray and physical examination
Hello there, I have a question about the medical exam that we need to do after receiving ITA. I have read that the form IMM1017E needs to be provided to the hospital which will be given by the visa office, as mentioned in CIC site. I would like to ask if that is already included once ITA is already received or do we need to do something to have that form? I am a bit confused, please help.

Hi, you don't need to take any form with you. Just tell the it's an upfront medical exam for PR-Express entry

Just book an appointment with CIC approved physician, while booking appointment u can ask for docs u need to get.

Mostly they ask for passport, xerox of passport, passport size photos, fasting NOT needed, mostly u dont have to take any ITA proof.

Just say its premedical check for canadian express entry PR.

share if u r FSW/CEC/PNP....inland or outland.

cheers,
Rrk.
