Medical Check Up Inland Sponsorship

Hi..I just wonder if I submit Sponsorship Application Inland , do i need medical check up first and submit it together with the application ? Or I must wait instructions from CIC ?
My wife wants to sponsor me . Do I need to extend my tourist visa while I'm waiting for my application ? Please give me advice since I apply it by myself and I can't call CIC call center because always busy. Thank you ;D

1. Yes, read the instructions, you must submit a complete application which includes the medical.
2. Yes you should apply for an extension.
