This refers to the Application to Sponsor a Member of the Family Class you submitted to this office on behalf of my (name po). We are not able to process your application as it is incomplete. You are missing the following:

• A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures for the person(s) you are sponsoring. This must be received at this office by: 10/20/2012
• A newly completed Sponsored Spouse/Partner Questionnaire (IMM5490) bearing an original signature for the person(s) you are sponsoring. This must be received at this office by: 10/20/2012
• A newly completed Sponsor Questionnaire (IMM5540) bearing an original signature for the person(s) you are sponsoring. This must be received at this office by: 10/20/2012
• An original Medical Report Form (IMM1017 Copy 2) signed by a Designated Medical Practitioner (DMP) for the person(s) you are sponsoring. This must be received at this office by: 10/20/2012
• Passport size photographs (photographs must conform to the specifications outlined in the Guide to Immigrating IMM 3999 and Appendix B Photo Specifications) for the person(s) you are sponsoring. This must be received at this office by: 10/20/2012
• Valid passport or travel document for the person(s) you are sponsoring. This must be received at this office by: 10/20/2012
• Please provide our office with a marriage certificate for yourself and the person(s) you are sponsoring This must be received at this office by: 10/20/2012

All requested documents/information and a copy of this letter must be submitted within the timeframes specified above (formatted as “month/day/year”). If you are unable to provide any or all of the requested documents/information, please explain why they are not available.

How come po na namiss niyo itong mga documents na to?
Mga important forms pa naman yang mga yan...
Hindi po ba kayo gumamit ng Document Checklist? Like ung IMM 5491E and 3905e po?

Anyway, diba sabi niyo po, approved na si sponsor niyo?
So, tong email na to, galing na po ba ng Canadian Embassy, Manila?
Pero ung passport niyo, pinadala niyo po sa Canada?

malamang yan sis CIC nag request nyan kse for step 1 lahat yan forms na yan at requirements. ang dapat lang malaman yng next step nila kase nawala na sila sa process kse naisend na nila yng original passport sa step 1 inquiry na yan dapat sa CEM kng mag PPR pa sila

Kung step 1 pa lang talaga to, dapat po photocopy lang po ng passport ang isesend kasi un ung asa checklist ng step 1 po. Either mag-antay po kayo until mag in-process kayo or mag-email siguro kayo to inform them na nasend niyo na ang mga missing docs na hinihingi nila at the same time macoconfirm niyo na kung natanggap nga nila.

That's all we need to know now . . . someone with past experience dealing with CEM please?

hello po. ewan ko sa embassy po bakit hinngi nila ang passport ko eh stage 1 panaman yun. we were just following sa kanilang request po kaya pinasa na lang namin eh hindi naman nila sinabi kasi na photocopy lang. cge lang maghintay lang kami. sana di mawala passport ko. ang nagrequest nito sa canada pa po.

Oh crap! The Appendix B on CIC's website says "two", although there are also some other minor differences, so I was taking that lightly.
