The question is;

In form Additional Family Information IMM 5406 (04-2008) E,
In section A;
What did you write when you filled the form and what did you write when your spouse filled the form?

First Column is

Name
(Applicant)
(Spouse or Common Law Partner)

Did you write your name in both forms or you wrote the name of the person who filled the form (Principal Applicant/ Spouse)

Please consider it a serious question and answer ASAP.

The instruction GUIDE states: This form must be completed by:
You, as the principal applicant,
Your spouse or common-law partner (whether accompanying you to Canada or not), and
Your dependent children aged 18 or over (whether accompanying you to Canada or not)
If you are single, there would only be ONE copy to fill - your own.
If in a relationship, your partner/spouse would ALSO fill out their OWN copy with THEIR personal name as Applicant, you as spouse, THEIR brothers and sisters names, their parents, etc
If children over 18, they will complete THEIR OWN copy

I hope Dr H will back me up on this, but I know this question has come up before. You can also do a SEARCH on the main forum page, top right corner. All the best.

in case of additional family information form----- dont b confused with applicant and principal applicant
in this form applicant doesnt mean principal applicant

when principal applicant is completing the form he is applicant and he will put all the information of his and sign and date it
when spouse is comleting the form she is the applicant and declare she hav child/or not,her parents name etc.that means each family memeber who is filluping this form he is applicant and gives his bio data and sign and date it. t
his form must b completed by all family member either they intend to go to canada or not if his age is over 18

when u fill up the form for ur spouse then he or she mention in the form : APPLICANT
and urself is ::spouse or common law partner..

Yes absolutely agree with you,

This question arose when a member wrote his name (principal applicant) on both of the forms on the applicant column.

And despite that member got PER From CIO.

The file is now with the Visa Office and I don't know how they are going to overlook it again or what consequences could be.
