Good day everyone,

My wife's papers were received by the Manila visa office in late May 2013 (I live in Canada) and they contacted us earlier this week saying they need the following documents within 45 days.:

Your original and valid passport.
Fully Completed Appendix A (attached)
Personal history from February 2013 to present(see attached form--include jobs held, periods of
unemployment, periods of study and any other extended use of time, such as time spent travelling
in search of a country of refuge, stays in hospital, prisons or other places of confinement, as well
as periods spent at home as a homemaker. PLEASE DO NOT LEAVE GAPS, AND USE
MONTH-YEAR FORMAT.)
Complete history of residential addresses from February 2013 to present (PLEASE DO NOT
LEAVE GAPS, AND USE MONTH-YEAR FORMAT.) (see attached form).


The strange thing is, the only form that they attached to the e-mail was the Appendix A. Where are the other attached forms that they mentioned, or where can I get them?
I would any assistance anyone can provide. Thank you... salamat po.
 
Reply to their email and ask them where the missing forms are. When I got that email last August I also had a question for them regarding Appendix A. I emailed them and they responded the following day. Good luck.

I assume you completed and submitted form IMM5669 with your application? Looks to me like they're looking for an update of your wife's residence, job, other activity since you applied in May 2013 (they're going back 3 months from that). Same with Appendix A, from all accounts it's information they should have but maybe they're just looking for a "refresh". Makes me want to to just submit a voluntary bi-monthly update on all this info so they don't waste more time asking for & then processing it. As to what "form" it is, very good question. They should have provided it...or maybe they'll accept a summary note from you/your wife?

Yes, a completed IMM5669 was sent last year and the timeline ended at April 2013 (which was when the forms were filled). Now they're asking for "February 2013 to present" information, so I'm not sure if I should just resubmit an updated IMM5669.
I've e-mailed the visa office and I'm hoping they respond by next week for clarification.
