Yes that's it. Make sure to write spousal sponsorship on the envelope and write inland or outland also.

Just to clarify, we do not need to include the fax line? I know this sounds pedantic but I was not going to include the fax line on the envelope. In the instructions it does not list the fax beside/below the address anyway.
