LOOKING FOR HELP TO FILL SPON APP

Hi ,

i am in process of filling application forums to sponsor my wife. some question that i am confused and need help.

1) Question 9 on IMM5540E asking ( what date i become permanent Res ). should i put the date i landed to canada which is july first 1996 OR the date of issue which is June 14 1996 shows on immigrant identification paper ?

2) Question 10 on same forums (were you yourself sponsored to Canada ). the answer is yes but i was sponsored by family member but i was under age of 18 and came along with my family .so what i should write ( dependent child, parent or any ) and what date ?

3) Question on IMM1344E - I am a naturalized Canadian citizen but date of obtained my citizenship card is only the year not month of day and the number is only 7 digit ..

Any help would be highly appreciated..
