Dear Noble, Do you mean you just got the MR forms? because I remember you posted before that you get MR,RPRF etc about a week or two weeks ago??

My 3rd line " your medicals have been received" added in 15th July (the day before yesterday).

Just now I have received an e-mail from VO stating

"Thank you for submitted the requested documents. In addition to the documents you’ve submitted, the following information is required:

- Supplementary Information form. This file is attached to this email and must be completed by all family members over the age of 18."

The attached form contains

PARTICULARS
1. SURNAME A) NOW
B) AT BIRTH
C) OTHER SURNAMES
Indicate sex male or female
2. FORENAME(S)
3. DATE OF BIRTH
4. PLACE OF BIRTH
5. CITIZENSHIP
6. FULL ADDRESS/ES FOR LAST TEN YEAR PERIOD (including present address)
7. OCCUPATION/S FOR LAST TEN YEARS (up to and including present occupation, full details)



I never seen the above format previously. Is it very common thing ? Is it like additional Information chart or PPR1? I am bit worried. I submitted updated schedule A form then what is the meaning to ask the same information again?

Awaiting for reply.
 
Finally, I received PPR this morning. I have also been able to submit my passport at VFS London this afternoon. The email contents are the same as what was described by Hila01.
