If one is working on a visitor permit, what are the consequences of
1. The visitor
2. The employer

What if the employer was paying cash?

Still the same. People are often caught working illegally when someone else reports them who is upset by the illegal work.

If you are caught and removed, expect it to be extremely difficult to return in the future on any sort of temporary visa.
 
This individual was working in a sushi restaurant and was being paid under minimum salary. The individual was suggested to cease working for this business due to a medical condition. I am curious if there is some legal loophole with an extension of a work permit.

No legal loophole if they were working Illegally. If they have already left the country then I'm not sure why any of this matters at this point.
