Last name issue

Good day all,
The last name on my application is different from my last name on myT4 slips.
On my application my last name is Feng Lee, and on my T4 is only Lee.
is that a issue with my application if my last name is different on my T4?

Awesome, thx Andy cause i was really worried that it might mismatch because of my last name.
I was thinking as long my date of birthday matches it should not matter.

T4 is issued by your employer. You employer will also issue the Reference letter on your name. You can also submit pay stubs as additional docs. So your case, is good enough. No worries.

Also your SIN is printed on your T4. The SIN should match the SIN on your Notice of Assessment, which should have your accurate information. If the SIN was different between your T4 and NOA, then that would raise questions. Otherwise, I don't think this should be a significant issue.

Andy and Seton thank you guys for the quick response, yes my sin # has my full name and i have only one sin #. I was just wondering if would raise any issue because of a last name =)
