know about my previous status

i applied a tourist visa through consultant 1 year before by myself but my passport has been collected by my consultant by sumhow I collected my passport from my consultant and my passport was blank and my consultant didn't gave me any reason for the blank passport, Now I want to know the reason for previous status.

Please guide me in this regard.
