I got the same email regarding background declaration form (IMM 5669), my question is

under section 12. Addresses
List all addresses where you have lived since your 18th birthday or the past 10 years.

i worked in Canada 2011 - 2012, 2013 - 2014 should I list my addresses in canada also or this is only for addresses outside of Canada????

We went to dr jason tsia. He is on cic drs panel. Not sure about his last name spelling. Staff was
professional and friendly. They provided us with form to fill out. We were done within an hour. Then went to x-ray walk-in clinic.
