Hi,

I did upfront medicals for cec july 2014. Today I received MEDICAL REPORT CLIENT BIODATA AND SUMMARY(IMM1017). What should I do ?

I did my medical upfront also but I didn't get anything like it. But tell me who send this IMM form by CIC or ur Medical panel doctor? please specify in more detail, if I can help....

I got the same email regarding background declaration form (IMM 5669), my question is

under section 12. Addresses
List all addresses where you have lived since your 18th birthday or the past 10 years.

i worked in Canada 2011 - 2012, 2013 - 2014 should I list my addresses in canada also or this is only for addresses outside of Canada????

This form must present to the Panel clinic when presenting themselves for thies IMEs. Filled both identities IME number & UCI number also.- as per CIC website

You have to list all the address where you lived since your 18 years old or past 10 years.. so i would say give details for last 10 years if you are over 28 years old.
All the addresses from canada and as well as all the countries where you lived over that period.
