Yes they have decision made on Sponsor’s side of Ecas when sponsorship is approved. A sponsorship approval email is the legit confirmation though since there is a very very tiny percent chance that the decision made is not in favour

not even tiny . they can't make decision without notifying , CIC always give 30-60 days notice in case they have some doubt . So this is firm Sponsorship approval .

I think so too. They wont reject if they have any doubts. If they have doubts, they will ask for clarifications.

Yes that’s what ECAS is referencing. You’ll also get an update on ECAS when they request the medical as shown below:
