ITA form filling doubts

Hello All

Trust you are doing good.

I have received an ITA for Canada recently. I have a few queries related to filling the form.
Please help me answering them:

1. I am being asked in ITA form whether I have a provincial nomination. I received an Ontario nomination but I haven't submitted it yet. Should I answer as yes or no ?
2. I have recently switched my job. This new job isn't mentioned in my Express Entry application. Should I show it in my ITA application? It won't effect my marks
3. I am being asked for all my relatives. What all relatives am I required to show here? What would happen if I miss mentioning father's name as my mother is divorced?
4. I am asked all locations I have stayed in past 10 years. I have lived in different states for work, but my permanent address has been same. Should I mention only my permanent address or the local addresses as well where I have stayed? I don.t have address proof for other locations now.
5. I have sown 4 noc codes for my job, in the form they are asking for primary code for which i am applying. The primary code is my current field of work. But my total experience in this single code is 2 years out of total 6 years. Will it decrease my application score?

1. Yes. Is this nomination from an Express Entry-linked stream?
2. Yes.
3. It should tell you who to include: spouse or partner, dependent children, parents, their spouses or partners, your siblings, etc.
4. Give every address you have resided at.
5. No.

Thanks for your quick revert.
1. Yes, it is for an express entry linked stream - Ontario immigration nominee program. I have received a notification of interest, but I suppose till I submit the form and get an acceptance I guess I won't officially get a nomination. Am I correct in this ? If so, then I guess I would have to answer the question as no. Please clarify.
