2. ADDRESS
3, NAME OF ACCOUNT HOLDER
4.BALANCE OF ACCOUNT HOLDER
5. SEAL OF BANK AND MANAGERS SIGN.


PLZ TELL ME IS IT MANDATORY TO WRITE BANKS PHONE NO. ON IT, AS I FORGOT TO WRITE IT
 
Don't worry, they can get it from Head-Office if they need.

If you getting an account certificate,I guess the bank must have given it u on a bank certificate.In that case all the information about the branch is mentioned on the letterhead itself.

If that is not the case why don't u ask them to give it on a letterhead.That will make it look more authentic and add weight to your case.

I am telling you what i have done.
