X-ray is mandatory. Your wife can do the physical exam and blood tests now, then inform the visa office she is pregnant and will do the X-ray after delivery(give due date). Or if you decide to go ahead with the X-ray, a lead apron will be placed on her belly. I don't know if it protects 100% against the rays.

You should consult with a doctor. It all depends on the body structure and how advanced pregnancy stage is.

Right about consulting a doctor but the choice to actually have the X-ray while pregnant is hers to make. The doctor will tell her about any risks, etc but can not tell her what to do.

Are you saying that you received the medical request without the attached "Section A"? Hmm... in that case, forward the email you received to the visa office and ask them to kindly send the attachment. Also, since time is getting tight, print the email and take it to a Designated Medical Practitioner in your city and see if he/she will do your medicals.

If spouse is pregnant, are we supposed to let the Immigration know? i though we only had to let them know once the baby arrives & the birth certificate are issued??

You do not have to inform CIC of your spouse's pregnancy. However, if they send a medical request and you decide to wait until the baby is born before she does the X-ray, then you will need to inform the visa office of your plans. Once the baby is born, you have to inform CIC.
