Is this Medical Examination Document okay?

Hi,

I recently got my Medical Examination done, after receiving the ITA.

The center gave me a document which looks more like an acknowledgement than an actual IMMI form.
They also said that they'll upload the reports directly to Canada Immigration's portal.

Is that how it works?
Was I not supposed to get an IMMI form from them?

Is this document good to go ahead with?
