Is option C also know as RC143- 2 E ???

Hi,

Just received my option C in the mail today.
But nowhere on the letter does it state Option C.
Just wondering if its the correct document.
On the bottom of the page there is a footer stating
RC143- 2 E.
Is the the correct document to be sent along with my T4?
