IRCC NEW portal has builtin family information page
you don't need to attach any additional forms
every thing is builtin in the portal to be filled online

Hi @holygrail15 , did you get the visa without uploading imm5645e? I am from Bangladesh. It appears that our visit visa will be processed at CHC Singapore.
