Hello,

I searched the forum for an answer before asking but couldn't find it. for example:

The name on passport is: John A. M. Smith but the instructions clearly say do not use initials, what should be using for the Surname and Given name?


Do I use John Smith without the initials or Do I use John Albert Mario as the given name and Smith as the lastname for IMM5257E?


What about schedule one?

Do I use the same or initials.


Sorry if this has been answered before but I tried to search as much as I can. Appreciated.

Write in the main boxes the name as it appears in the travel document.

Then in "other names" write the complete name without initials.

Thank you very much, I thought about that but the file wont take an initial with a . , I am just worried that they are too picky :)

As long as you make sense in the application you shouldn't worry.
 
Given name - John Albert Mario
Last Name - Smith

Hope Clear..


Lots of same situation solve by same answer...but name are different.. :P...
