I was accepted into the express entry pool with a CRS of 378, the response I got states ineligible for express entry. I have my masters, passed my IELTs, have over 3 years of full time paid working experience, my degree verified by WES. I want to reapply, but I do not even know what I did wrong to begin with

Are you eligible with at least 67 points (not CRS)? What is your IELTS scores for each band and age?

IELTS, L-6.5, R-7.5, W-6.5, S-7.5 overall 7.0, age 33.

See what your score is and if over 67, then you should be eligible but answering a question or two wrong.

I have a distant cousin in Canada (she is married and living with her husband and son in Alberta), but the relative's options doesn't include cousins. if not it would have made up to 67 with her 5 points

The 62 points is the reason you don't qualify. You'll need to get your score to 67 points to qualify.

Start by re-taking IELTS and achieving a higher score.

The counsin doesn't help you. That's too distant a relative to get any points.

If it's a PNP stream through Express Entry, then you are not eligible.

The only province since you don’t have a job offer is Saskatchewan. You would need to calculate your SINP points and see if your NOC is drawn.

No, you need an eligible EE profile.
