Indian Birth Certificate Fromat question !!need help

Hello Guys,

I am hoping some one who have sponsered their partner from india can help me with this question.

My file is returned twice and first time my agent did not fill out the case specific form which i did second time and send it back,

Now I called CIC agent as its been a month i was told it's returend again 3 days ago due to identity document ,My question is what would be the format of the birth certificate should be,


Mine was attested and translated it was done in Ludhiana city.

It should be done from the same place of birth . Or do it from chief medical officer of the place of birth city.
