India Consent Form VFSGLOBAL..Need Help (Anyone has filled it)

I have few doubts regarding the consent form.

1) Name is written as printed in the bracket. So what does that mean.

2) My mailing address is different and my passport address is different. So what should I write?

Hi - did you get the answers to your questions? I have the same doubt...........please let me know what you did and did it work?

Printed in bracket just means you have to put the same name as in passport. If your mailing address is different then put that address in the both the forms as i did and it was not an issue.
