Incorrect Personal Particulars Proforma for PCC from Indian consulate in SFO?

Hi, The Personal Particulars Proforma questions doesn't make much sense, it appears like it's a form to apply Indian Passport. I am following instructions from below link:

My question is about 8a, 8b, 9, 10. I am currently in USA, so in 8b do I need to fill my USA address? If it's USA not sure what details to provide for Police station? Also do we need to provide references from India for question 10?

At the bottom of the form it say's RECOMMENDED PASSPORT with YES/NO option, which makes me wonder if I am using correct form here. Any one who filed for PCC recently using this form? I am guessing this could be a wrong attachment in the website. Want to make sure I am sending correct form here. Any help will be greatly appreciated.

P.S: Indian citizen living in USA applying for India PCC for Canada PR.

I am applying for India PCC through embassy since I am in USA and in form "Personal Particular Form" , in column 8A - should I fill US address and 8B - I can fill India permanent address but none of these address would be the address at the time passport was issued.

Can you please share your experince?

Hi Ssen11,

Are you required to fill personal particular form to get PCC from embassy. I had to fill because there was no passport clearance report on my passport in India for my regional passport office.

Can you please mention what you need to know so that I can answer accordingly?

I am applying for Canada PR and currently reside in USA since 3.5 years.My question is what should I fill in 8b).Should this my India address or my US address?
