For the sponsorship evaluation form, do i print the form so its on both sides of the paper? Or do i print the form out as two separate pieces of paper. I only ask because question 6b says to provide details on the back of this page which is the second page.

We printed everything single-side because they tend to scan everything, as I understand it. They might miss something written on the back of the page.
