IMM5709 (VISITOR--> WORK VISA--> applying Study Permit)

I am applying for Student permit and filling in IMM5709 form. I have confused with only one question under Background Infor. regarding my condition.

2c) Have you previously applied to enter or remain in Canada?

My condition: Approved work permit with LMO - August 2013
Approved extended visitor visa - May 2013

;D So....DO I HAVE TO SAY "YES" for this question? AND how to state the details?

I currently got admission from University and would like to study SOOO much.. Please help me if there is anyone knows about this..
