Hi guys, I'm just a bit confused what to answer for IMM5669 "had any serious disease or physical or mental disorder?"
I have ICD(implantable cardiac defibrillator)/pacemaker for my congenital heart condition, but my condition has been stable with no medical episodes for past 10+ years, just two surgeries so far to implant and change the battery of the device in the span of 10 years. My condition doesn't really affect any daily activities, unless I compete in sports (which I don't).

Should I say yes or no to that question? For now, I said yes and I added "Diagnosed with Long QT Syndrome type 3 in 2011; managed with an implanted cardioverter-defibrillator (ICD). Condition has been stable since 2012 with no impact in daily activities."
If yes, do I need to get some sort of medical statement proving my explanation from the specialist and add it as a supporting document?

Another question is do I need to follow the document checklist only or should I add any supporting documents/application forms that may be applicable (i.e. language test result, proof of fund, proof of education, etc)?
 
Yes, you should reply "Yes". Provide all your medical details and supporting documents.

The second question is not quite clear.
