my ex husband has immigrated to canada recently and is applying for residency. i live in lebanon and have custody if 2 kids. he has asked me to sign the 5604 imm form saying it is part of his visa process. is this true? is this declaration form related to his stay in canada or is he planning on kidnapping the kids
 
Are the children going with him? This form is only if he plans to take the children with him to Canada, it is to get your permission for them to travel.
 
no the kids are not with him. he says they have requested this form as part of his visa process. so it will not affect his stay in canada of i dont sign it?

This form should only be necessary if he plans to take the kids to Canada without you. It is not needed if he is applying for residence for only himself.
