IMM5556 - What if I have LMIA when I'm applying for OWP?

hey guys. I have LMIA and closed work permit(issued this month) . I am about to apply for open work permit with common law PR application. I was filling up the IMM5556, there is a checklist that says "If your employer obtained a LMIA from ESDC, provide a copy of the LMIA and a copy of your job offer letter from your prospective employer "

should I provide LMIA copy and job offer letter ( I got this last Dec) from my current employer? just it says prospective employer. I was confused

No - don't include the LMIA or job offer letter. You are applying for an OWP.

Appreciate it! Just one more questions, when common law application forms require to write the date of common law relationship started, should i write down the declaration date?(which is yesterday) or the first date we started living together?
