Q: do I need write my wife or child's name? I have not any previous spouses or children.

Part C - Info about Relationship

3. Are you living together now? Yes or No
Q: My wife and me live together for over one year before I came back to Canada in August 2016, so should I answer Yes or NO for this question?

Q: my wife will accompany with my one year old child to Canada together, both of us are legal parent of my child, so it means we do not need to file the Declaration from non-accompanying parent for minors immigration to Canada. is that correct?

Part A - 3. You need to list anyone that is not included already included in the app but that is financially dependent on you, i.e. a Canadian child, parents you completely support etc.

Part C - 3. Pretty self-explanatory. Are you living together now?

Checklist - You do not need that declaration. It is only required when one parent is immigrating with the child while the other parent is remaining in the home country.

so what about part c question 4? "if you are not living together, have you ever visited each other during your relationship?" its a yes or no question.. at one point we lived apart but we live together now and have since early last year and I can't uncheck that question.

Your answer to question 3 is yes - you are living together now. They then want you to list the times you have stayed together since the relationship began.
Question 4 says 'if you are not living together...', so since you are living together you do not have to answer it. If the form will not allow you to skip the question, put the times you visited one another in the space provided.

Who should fill out part C of this form, should the sponsor fill it out or the Principal Applicant?
