Hi,

In form 5490, there are some points like 1) "6. Additional details describing the circumstance of your first meeting with sponsor?"
2)"9. Describe how your relationship developed after your first contact with your sponsor and if you and your sponsor dated or went on any outings or trips together. Provide photos and documentary evidence of events or activities in which you both participated".

I have added more information on separate sheet.

In addition to this, Do I have to provide separate letter to explain development of relationship. Because some of writing is repeating.

I didn't provide additional letter. My sponsor did on his end, but I just explained all of the answers on my questionnaire in great detail (took about 3 extra pages altogether), and I think that's fine.. as long as you have all the information in there somewhere! The 'story' of our relationship was told in answering the questions. Other people attach an additional explanation, but I don't think it's required if you answer the other questions thoroughly.
 
I had 2 pages for the 6) First Meeting. Underneath, I wrote "Please see Proof of Relationship:" and listed where the photos were. For question 9) Development of Relationship I did a few things. I put a timeline first from first contact all the way to today. And then I had a 10 page novelesque detailing of each event. They did not have to read it since they have the timeline, but if they wanted more info, it was there. Underneath each section "Applicant Visits Sponsor (date to date)" for example, I showed the IO where to find the evidence.

Now on the form, they ask you for a letter. I'm sure you don't have to include it if you already answered, but I wanted to comply to the instructions in an obsessive manner and did include an additional letter, one page. I did note that my sponsor already said most of it as well as my development of relationship information. The letter was very mushy, very poetic because I'm a writer... :) ...but I doubt that's necessary. Either way, that's what I did!
