Question 4 asks how many family members are included in the application and the instruction guide says to include my family members, whether or not they are accompanying me to Canada. As far as I know, with the form Additional Family Information I list my mother, father, and sister as family members. So with all of them including me it's obviously four, but is this exactly what they're asking for with question 4 on form IMM0008? None of the family members I listed in the other form will be accompanying me to Canada ever and I don't even speak to my father. Does it just refer to myself?

I'm also not sure if I have my father's date of birth right. I'm fairly certain I do, but it could be a little off. My mother doesn't remember his date of birth and I'm not on speaking terms with my father, so I can't ask them. Can I just write a note explaining that there's a chance his date of birth could be wrong or should I just leave it?

They referring to core family; spouse & children. Not parents, aunts, uncles, etc. So, if it's just you, then 1. If it's you and a child, then 2, and so on.

As per another thread; Depending on the country, your fathers D.O.B. may be listed on your birth certificate?

That's what I thought, thank you!

It's not on my birth certificate, unfortunately.

As a personal opinion I would write in 'unknown' as birthdate and then add an extra sheet (properly labelled of course), and explain exactly this. You suspect it's such and such a date, but have no way to confirm it as he's not part of your life. You're definitely not the only case like this; even just earlier today someone had the exact same issue, so I'm sure CPC-M is well aware of situations like this. Best of luck.

That's a good idea. I hadn't thought of writing unknown. I'll probably go ahead and do that.

Thanks again. :D

To confirm, you will have no issues with this. We didn't even know if my partner's parents were still alive, let alone their addresses, birthdates etc. Just include an additional sheet explaining that you don't know because you have no contact with him.
