IMM 5669

I got a mail from CIC today asking me to submit the IMM 5669 for my spouse. Basically what is this? Can anyone help?

Schedule A: Please provide a separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature.

 This must include updated personal, address, and travel history since the age of 18 with no gaps in addresses or activities. In addition, ensure that you provide responses to all statutory questions as well as additional explanations for all statutory questions for which you have provided a response of 'Yes'.

well, they have very clear instructions in the email itself. Find form IMM5669 on cic website and fill it in since the age of 18. Then print, sign, scan and upload.
The only thing I am a bit surprised is that they ask for travel history... there is no special box for this in schedule A, if i am right... I guess u would have to make a separate table for travel history and send it along with Schedule A
